Read this before you buy

This covers appeals under the ACA's federally standardized process — marketplace and individual plans, and employer plans including self-funded ERISA plans. That's most of the coverage people have.

It does not cover Medicare, Medicaid, short-term plans, health care sharing ministries, or grandfathered plans. Those run on different rules with different deadlines, and a guide claiming to cover all of them at once would be too vague to use. This one goes deep on the largest group instead.
